Job description

The successful applicant will apply contemporary speech pathology skills and knowledge in the assessment and treatment of clients within the Residential Transition Care Program to promote, maintain and improve their health, functional ability and quality of life.

About the role

  • Transition Care supports older people who have been discharged from hospital or a subacute facility to undertake a time limited low intensive therapy program to help improve general function and overall independence and to make an informed choice about future accommodation needs. Transition Care supports the older person who may need more time to decide on their long-term aged care options.
  • Working hours are from 0800-1630 Monday to Friday, with some variances in start/finish times dependant on the clinical alignment. Further extension of operating hours may occur based on client and hospital and health service need.
  • In future, this position may be required to participate in weekend and public holiday rosters depending on the service requirements.
