About the role
Specialist Palliative Care services incorporated into this role include managing patients across inpatient, outpatient and community Palliative Care services in West Moreton Health.
The role will be spread across the above services and with duties allocated to where there are specific areas of need.
The Palliative Care Community Service works in an integrated model of care with the Palliative Care inpatient and consultation services and consists of a Home Visiting Service (including face to face visits and telehealth), Palliative Care Hospital in the Home Program and Specialist Palliative Care in Aged Care Program. This position also supports the three rural hospitals in the health service to deliver high quality Palliative Care.
Specialist Palliative Care services in West Moreton are delivered through an integrated model of care with all aspects of the service working collaboratively toward patient centred care across the continuum as the patient moves seamlessly through different settings of care. The inpatient/outpatient service provides Specialist Palliative Care consultation and advice for inpatients, patients in the emergency department and outpatient clinics.

About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated interpersonal skills which allow effective communication in a multi-disciplinary environment.
- Demonstrated knowledge of safety and quality assurance initiatives, as well as the ability to conduct audits and data analysis of key outcome measures.
- Experience in relevant areas of inpatient and outpatient care.
- A sound knowledge of contemporary Human Resource issues including time management, Workplace Health & Safety, Equal Employment Opportunity, Anti-discrimination and Code of Conduct
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
