Silverchain Palliative Care Service has a vacancy for a Registered Nurse, based out of our Birtinya Service Delivery Centre providing services to the Regional Council areas of Gympie, Noosa and the Sunshine Coast. This role offers the opportunity to work in the community setting operating 7 days a week.
Requirements are Monday - Friday a little weekend with shifts either 7am - 3pm or 8am - 4pm day shifts, no afternoon or night shift which will help you to facilitate a work life balance!
Key ResponsibilitiesWorking in an integrated multidisciplinary team, you will be accountable for the provision of high-quality Palliative Care in the comfort of patients' homes. You will develop and implement individualised care plans that are inclusive of education and emotional support that empowers the patients and their families/carers' on care strategies and end of life issues.
You will take a compassionate and empathetic approach to Palliative Care assessments; maintenance of care plans; monitoring and managing symptoms; addressing pain and discomfort; supporting physical, emotional and spiritual needs for patient and family/carers.
About youYou will have a general interest and passion for Palliative Care along with:
- Nursing qualifications and current APHRA Registration with no restrictions
- Minimum 3+ years post graduate clinical experience in Palliative Care / end of life care in the community, hospice or inpatient setting is required.
- Recency in a Palliative care setting
- Broad range of clinical skills & competence including but not limited to the areas of medication management, syringe drivers, rocket drains
- Strong communication (written and verbal) and interpersonal skills
- The ability to work autonomously & in a team
- Full Australian Drivers Licence
- Based on Silver Chain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19, or evidence of a medical exemption in order to commence employment with Silver Chain.
