Job description
- Assist the client to develop person centred care plan, behaviour management plan, review the plan according to organisations process and procedures.
- Documentation and record keeping and storing of client information,
- Assisting client with personal care, general hygiene, dressing and supporting clients through programmes to meet clients goals. The level of support provided depends on the client’s needs.
- Providing domestic assistance: cooking, food preparation, household cleaning, laundry and ironing tasks, assisting people to perform important daily activities.
- Social support: grocery shopping, socialising, support and advocate the client to encourage to meet their personal develop care plan.
- Providing Transport Services to medical appointments or community access and to ensure the client is safely driven around, have appropriate aids to assist with disabilities.
- Review and monitor clients progress and update client records accordingly on a daily basis.
- Encourage the use of new aids to develop the clients intellect and communication skills where appropriate
- Emotional support, Dementia and Respite Services
- For experienced and qualified carers, opportunities are also available providing complex care to clients which high level needs across disability support.
- Ensuring a varied program of social activities
- Providing emotional support and friendship, help deal with anxiety, depression and any other disorders affecting the client, to help the client. seek professional services to be included among those you are already receiving.
- Encourage development of independent skills and utilise learning to allow clients to develop where possible.
- Prompting and Supervising Timely and appropriate medication or contact medical services for assistance.
- Developing Personalised Support Plans by fulfilling individual goals, needs and requests. To ensure that every client’s needs are met, and the funds disbursed cover the necessary services.
- Identify hazards and carry out risk assessments ensuring the safety of employees and clients.
- Follow and comply with organisation polices, process and procedures at all time.
- Ensuring working in compliance with WHS legislation and standards
- Working with Partnering Health Workers, family to ensure health, educational goals, study are met.
- Any other tasks required to carry out your role safely and to meet the needs of the client.
