This position is responsible for the delivery of quality services to Participants who are accessing Support Coordination from the NDIS. Providing quality service provision, stakeholder engagement and day to day support to team members.
- Engaging with Participants, their supports and third-party organisations in a professional manner
- Working with Disability Support Workers and Participants to help build their own capacity/independence
- Working with Participants to achieve their goals in line with their NDIS plan
- Provide supports of a complex nature including mealtime assistance, manual handling and personal care
- Ensuring Supported Independent Living services are provided within the principals of the NDIS
- High standards of ethical behaviour, disability knowledge and transferrable people/support experience
- Responsible for mentoring and monitoring staff, ensuring they are equipped with information, instruction, resources and training required to work safely and deliver quality supports
- Able to assist in a crisis and be available for emergency response in regard to ensuring essential supports and services are provided to participants as agreed
- Ability to work independently, multi-task, and time manage
- Good literacy and numeracy level of competence, able to complete legal records such as case notes, emails, and use online software
- Experience in performing daily living skills such as budgeting, menu planning, shopping, and other domestic tasks to a high standard
- High level of communication skills, both verbal and non-verbal
- Work alongside the Coordinator to ensure that participant information is maintained and regularly reviewed including such areas as personal details, risk assessments, WHS checks, case notes, and Individual plans
- Ensure regulatory compliance and contribute to safe work practices
- Adhere to internal policies, reporting hazards, and always prioritising safety
- Any other task as delegated for the position
- Working with Children (Blue Card) and Disability Services (Yellow Card)
- Right to Work within Australia – Australian or New Zealand citizenship or relevant working visa documentation
- Successful completion of the ARC Introduction program
- Undertake pre-employment drug and alcohol testing
- Current Queensland Drivers Licence
- Comprehensive Car Insurance
- Relevant Qualification or Certificate with relevant experience, or relevant extensive experience in Accommodation Settings, Disability or Community Services
- First Aid & CPR Certificate (This will be a nominated First Aid Officer Role)
- Minimum of 2 Years’ experience as a direct Support Worker or equivalent
