What You'll Do:
Empowerment: Provide personalized support and advocacy to individuals with disabilities or mental health challenges, helping them navigate the NDIS (National Disability Insurance Scheme) and access the services they need to achieve their goals.
Collaboration: Work closely with clients, their families, support networks, and service providers to develop and implement individualized support plans tailored to their unique needs and aspirations.
Coordination: Coordinate the delivery of services and supports, ensuring they align with clients' plans and funding allocations, and advocating for additional resources when needed.
Documentation: Maintain accurate and up-to-date records, including progress notes, reports, and service agreements, in compliance with organizational and regulatory requirements.
