About The Role
The Support Coordinator will be required to provide unbiased and practical advice by understanding individuals NDIS plans and engaging with organisations offering appropriate supports for their needs. You will also assist them to resolve points of crisis and support them in achieving their goals.
Knowledge, Skills and Abilities
- NDIS Support Coordination experience is a must have and in particular specialist support coordination.
- Extensive knowledge of the NDIS, Disability Services Standards, regulatory framework, the disability sector in general, inclusive of person-centric approach and understand the importance of choice and control.
- Assisting participants to connect and coordinate their informal, mainstream and NDIS funded supports.
- Responsible for support coordination of people with standard and complex needs ensuring quality services are received.
- Assist with resolving points of crisis, developing capacity and resilience within networks.
- Coordinating supports from a range of sources, liaising/ case managing with diverse providers.
- Responsible for providing progress reports and recommendations to NDIA.
- Demonstrated knowledge and experience with technology including Microsoft Office and relevant record management system.
- Allied health or social work degree mandatory requirement

- All applicants must be Australian Citizens / Permanent Residents of Australia.
- NDIS Workers Orientation Module.
- Current Senior First Aid and CPR Certificate.
- Current Working With Children Check.
- Current WA Driver’s license.
- Fully insured motor vehicle.
- National Police Clearance (6 months old or less) or willingness to apply for and provide prior to commencing employment.
- Applicants will be required to undergo a pre-employment medical.
