As a Support Coordinator you will be responsible for building relationships directly with NDIS participants, their families, carers and other service providers to implement supports/services that align with each participants goals and aspirations.
- Work with participants to provide ongoing support to reach their goals in line with their NDIS plan.
- Maintain accurate and up to date progress notes and participate in review and reporting processes with the NDIA and Local Area Coordinators.
- Stay up to date with the National Disability Insurance Scheme and practices associated.
- Execute all tasks given by the Thrive Support Coordinator Team Leader.
- Support your participants to understand and access supports within the NDIS.
- Strengthen and support a participant’s formal and informal support network of providers and ensure that services offered by these providers are person focused and tailored to meet individual requirements of the participant.
What you’ll need to be considered
- Current Drivers licence
- Blue card
- NDIS Worker Screening Check
- Great time management and attention to detail
- Strong interpersonal and communication skills
- Integrity, respect, empathy, and passion to make a difference.
- Personable and customer focused approach
- Ability to resolve conflict and de-escalate emotionally charged scenarios.
- Ability to travel throughout the Cape.
