Job description
At Carelabs, the role you will undertake is a hybrid position that combines support coordination and therapy work.
At Carelabs, you will work a minimum of 5 days a week on a flexible rotating schedule or fixed schedule. Approximately 80% of your time will be dedicated to therapy and clinical work, while the remaining 20% will involve specialist support coordination for individuals at Level 2 and 3. You will have the opportunity to actively engage with the community, develop personalised group-based therapeutic programs, and participate in respite activities.
This is a full-time equivalent (FTE) role specifically designed for an Occupational Therapist. Your responsibilities will include assisting participants with assessment, rehabilitation and providing education on how to maintain their independence.
Scope and Responsibilities
80% - Occupational Therapy / Assessments / Clinical
20% - Level 2 & 3 Support Coordination
Undertaking on-site and telehealth assessments for people experiencing disability across Australia.
Undertaking on-site and telehealth ongoing therapy for people experiencing disability.
Assess and write Functional Capacity Assessments for both physical and psychosocial disabilities.
Provide assessment, reviews and reports for Home Modifications and Assistive Technology.
Undertake Level 2 & 3 Support Coordination - Metropolitan and Remote Locations
Provide consultation to people with disabilities, families, carers, and other professionals.
Assist and co-organise therapeutic programs and workshops for people with disabilities and carers.
Attend networking sessions, disability conventions and workshops.
Undertake risk assessments and risk mitigation.
Ensuring duty of care and child safety practices are upheld.
Participate in multidisciplinary meetings, and provide treatment planning recommendations.
Ensure continued professional development and applying best practices.
Undertaking cross-roles such as marketing and sales.
Ensure continuity of care through referrals.
Abide to the NDIS, AHPRA and Relevant Legislations, and Internal Policies
