This is a full-time role that embraces contemporary practice, while working flexibly in a work/home hybrid model. We are seeking a candidate who will work proactively with NDIS participants by coordinating and engaging supports to ensure they are getting the most out of their NDIS plans. This role gives you the opportunity to foster and build strong relationships with participants, their families and carers. You will identify, connect, and negotiate with providers to deliver quality outcomes and timely service whilst working closely with other external stakeholders involved in an individual's care or provision of services.
To be successful in this role, you will have:
- Have 2+ years' experience in a Support Coordination or similar role.
- Qualifications in Social Sciences, Human Services or Allied Health (desirable)
- Experience with NDIS compliance requirements.
- Great relationship-building skills
- Exceptional organisation and time management
- Experience in monitoring and achieving billable hours.
- Excellent written and verbal communication
- Ability to work collaboratively with a wide range of stakeholders.
- Well-developed communication and interpersonal skills
- Passion and professional drive to continue developing your skill set.

- A current Working with Children's Check is essential.
- Full Australian working rights.
- National Disability Insurance Scheme (NDIS) Worker Screening Check
- A current driver's license and well-maintained vehicle.
- Completed the NDIS Quality and Safeguards Commission online Worker Orientation training module.
