About you:
Apply to join our team in Renmark, delivering exceptional care with UnitingSA. Provide tailored support, promoting safe, independent living in accordance with Care Plans. Document all provided assistance and promptly escalate any health concerns to ensure wellness and safety.
Key Responsibilities
- Provide personalised, goal-oriented care for independent living.
- Promptly escalate client issues to a Coordinator or On Call.
- Communicate openly with clients, families, and advocates.
- Actively engage in individual and group care activities.
- Effectively manage time and inform clients of scheduling constraints.
- Guide clients to their Coordinator for service changes.
- Tailor support for culturally diverse clients.
- Assist clients in communicating service changes.
- Monitor and report changes in clients’ well-being.
Skills & Experience:
- Previous experience in community home care with older people.
- Proven ability to communicate effectively with clients and stakeholders.
- Experience in catering to culturally diverse older people – a plus.
- Ability to work autonomously with minimal supervision.
- Proficiency in understanding and following client Care Plans.
- Track record of building rapport with clients and stakeholders.
- Essential minimum qualification: Certificate III in Individual Support or Community Services.
