About the role
The Community Services team in East Metro Perth supports people living with disability through community engagement. The difference you can make includes supporting our customers who are all very individual and have varying needs.
You will need to be prepared to support customers with personal care, going to medical appointments, to work and various community activities such as sports, walking, shopping and cooking. An outgoing, vibrant personality will see you achieve these outcomes. Please review the roster before applying.

Key duties include:
- Delivering a wide range of domestic supports within the home and community;
- Supporting customers to engage in community-based activities;
- Supporting person-centred plans that identify a customer’s life goals;
- Ensuring positive behaviour support plans are followed; and
- Administration duties and mandatory daily reports.
What you need
- Well-developed interpersonal skills with the ability to build effective working relationships;
- Ability to follow Person Centred plans and assist people to achieve their goals and outcomes;
- Previous experience with manual handling and medication administration is required;
- A willingness to assist with personal care and domestic duties experience;
- A willingness to use your own private vehicle to transport customers (aligned with Activ’s motor vehicle allowance rebate);
- Proven administration and computer literacy skills.
