Position Overview:

 

Dignicare Health Services Pty Ltdis a privately owned Western Australian company based in Perth operating sinceMarch 2022.

Dignicare Health Services offersvariety of NDIS support services to clients with special needs and ageingsymptoms.

Our team consists of experiencedmedical professionals and community service workers who offer services toenable clients to live their lives with more independence and ease. Servicesinclude group / community based activites, life skill development and support,personal and house-hold tasks assistance, nursing care, as well as travel/transport assistance.

We are currently seeking a skilledand experienced Support Worker- Coordinator/team leader to join us in a fulltime capacity. As a team leader your will be responsible for managing otherSupport Workers, planning and execution of care plans for clients,administratitive tasks as well as ensuring that all activites and care is providedin line with the organisational policy and procdedures, OHSC and Medicationadministration policies.

In your role, you will workclosely with individuals to identify their goals and develop and design supportsolutions to achieve participant outcomes

Responsibilities:

·        Lead the team ofSupport Workers.

·        Establish rapport anddevelop working relationships with clients and other service providers with an objectiveto achieve all elements of the care plan.

·        Support clients toparticipate in age-appropriate activities and tasks including employment,in-home support and community access.

·        Support clients to accessactivities and outings in the community.

·        Provide direct assistanceto each client in accordance with their service plan and as directed.

·        Carry out administrativeduties.

·        Work in accordance withthe medication administration policy, work health and safety procedures, andclient rights pol cies inclu ing privacy and confidentiality.

·        Monitor the safety andwell-being of clients, resolve and report any concerns and incident reports tothe Manager.

·        Ensure completion ofappropriate documentation. Relevant information on support provided is to berecorded in client notes and communication books as required.

·        Participate in theLifestyle Service Planning process for individual client and monitor outcomes.

·        Work with clients, family/carers, relevant case managers and other professionals to support the client'sindividual goals.

·        In conjunction withther members of staff, assist in developing, implementing and evaluatingtraining and behaviour program to attain identified individual goals.

·        Ensure that all programmingundertaken is appropriately maintained, documented and reviewed.

·        Assume the role of keySupport Worker for designated clients, including the preparation of a monthlyreport on client progress.

·        Liaise extensively withother staff to ensure overall smooth and consistent running of the service.

·        Actively contribute toa team environment through open communication, conducting regular staffmeetings planning processes and policy review.

·        Provide backup andsupport to other staff, as necessary.

·        Participate in PerformanceDevelopment meetings to identify ongoing training and support opportunities.

·        Contribute to Casereviews with Employment Consultants and Facilitators.

·        Carry out their rolesand responsibilities as detailed in the relevant policies and processes.

·        Keep fully informed ofthe contents of the NDIS system and apply it to daily work.

 

Qualifications:

·        Minimum Certificate Illin Disability

·        Current NationalPolice Check

·        Working with ChildrenCheck.

·        First Aid certificateand CPR

·        NDIS WorkersOrientation

·        100 Points of ID

 

Skills and Experience:

·        Minimum of 2 yearsrelevant experience

·        Previous experiencesupporting people with Disability.

·        Leadership experience- ability to lead, delegate and train others.

·        Demonstratedproficient written, verbal, and interpersonal communication skills.

·        Demonstrated abilityto work with minimal supervision, and collaboratively as a member of multidisciplinaryteam.

·        Previousadministrative experience.

·        Ability to effectivelyprioritize and organize own work/caseload.

·        Demonstratedcommitment to professional development, quality improvement, and evidence-basedpractice
