What You Will Be Doing
The Early Intervention in Chronic Disease (EIiCD) program plays an important role in supporting people with chronic and complex conditions and vulnerable members of the community.
The EIiCD program delivers service provision which is person-centred multidisciplinary care across all Community Health sites. Service provision should be provided in a manner that is well planned, integrated, and supports the person’s capacity to self-manage
The Team Leader is to work within the EIiCD program leadership team to facilitate on-going quality improvement in service development and delivery within the EIiCD program. As a clinician the Team Leader will hold both a leadership role and a clinical caseload as per clinical discipline.
Key Responsibilities:
• Undertake operational responsibility of the Frankston EIiCD team, to manage the delivery of high quality services on a day to day basis.
• Support staff by providing; operational supervision, actively promote and demonstrate the values of Peninsula Health.
• Facilitate on-going quality improvement activities in service development and delivery.
What You Need
We anticipate you will have:
• APHRA registered Physiotherapist.
• Well-developed consultation, problem solving, negotiation, conflict resolution and facilitation skills.
• Able to work as part of a multidisciplinary team.
• The ability to set priorities and monitor work demands.
• Excellent verbal and written communication and report writing skills.
• A current Victorian driver’s licence.
• Understanding of current EIiCD funding and service agreement requirements: HACC/CHSP/ICDM.

*Please refer to salary circular ALLIED HEALTH PROFESSIONALS (VICTORIAN PUBLIC SECTOR) (SINGLE INTEREST EMPLOYERS) ENTERPRISE AGREEMENT 2021 – 2026 for salary range. Sub-grading will be based on years of experience.
