About the Role:
As a Care Coordinator - Team Leader, you will play an important role in ensuring the team Provide and maintain high standards of all aspects of support and care where identified. You will be responsible for ensuring the establishment of all new programs as well as existing ones are running to their full potential, as well as driving the team forward and ensuring support is provided to the team
What we are looking for someone with:
Experience as a Care Coordinator or within a similar role
Solid understanding of the NDIS or disability sector
Strong analytical and decision-making abilities.
Effective leadership qualities.
Tasks will include but are not limited to;
Demonstrated ability to connect with people and prioritise building strong relationships across the direct care sector
Ensuring all leads received from sales team are delegated to a key Care Coordinator, with you overseeing that each program is established with the highest standards of service delivery. This includes risk assessments, care plans, staff meet and greets are organised by the designated Care Coordinator promptly and effectively.
Proactively engage with clients, ensuring their needs and requests are heard, understood, and efficiently addressed.
Foster a client-focused culture within the team, driving a commitment to exceptional service.
Successfully manage multiple responsibilities and tasks, demonstrating a keen understanding of priorities.
Liaise and delegate tasks to the team accordingly.
Excel in the setup phase for new clients, ensuring meticulous planning and organization from the outset.
Take initiative to streamline and optimize rostering processes, contributing to the overall efficiency of care coordination.
Exhibit a commitment to the longevity and success of client programs through careful attention to detail during the initial phases
Display a willingness to contribute to launches, initiatives, and support in different geographical locations.
Demonstrate flexibility and adaptability to work in diverse environments, contributing positively to the organization's growth.
Collaborate with coordinators and other stakeholders to address challenges and improve online care delivery.
