About the Role
The Carer Support Planning Team acts as one of the first points of contact for carers, supporting them through a registration and assessment process and subsequent coordination of required services.
Reporting to the Program Manager Carer Support Planning, the Team LeaderCarer Support Planning has overall responsibility for:
- Leading and contributing to a supportive and fast paced high performing team environment
- Liaising with carers and negotiating with service providers to put supports in place for high complex needs situations
- Developing expert knowledge of available internal services, partner services and services available through external organisations
- Using effective time management to ensure successful delivery on KPI’s and approvals to meet contractual and service requirements
The successful candidate will ideally possess:
- A demonstrated ability to influence and excite others to unite and move towards achieving common goals
- Interpersonal skills which are inclusive and encourage the development, cooperation and support of others and which emphasise and encourage a professional service delivery focus
- Relevant experience in a telephone counselling, and/or experience in an intake and assessment service.
- A demonstrated ability to work under pressure, make clear and quick decisions and work with clients in distress
- The ability to apply a person centred or strength-based approach and a consumer directed model of service provision
- Proven ability to analyse operational systems and process to support continuous improvement and drive innovation
- Strong verbal and written communication skills with the ability to negotiate to achieve successful outcomes

Successful candidates will need to have:
- A National Police Clearance
- Current DHS Working with Children Check
- Completed Child Safe Environments – Through Their Eyes training (or be willing to undertake this training)
