Position Summary The Transplant Ambulatory Care nurse provides holistic care to the patient and their carer/s throughout the transplant journey from pre-transplantation through to ambulatory care post transplantation. They will have the clinical expertise to assess, plan, facilitate, implement and monitor the care of patients in the transplant unit.
