The Role
- Make a difference in our community and to the lives of older people to remain living independently at home !
- We are seeking a suitably qualified and experienced applicant to lead and manage a diverse team
- to deliver community aged care services through the Support at Home Program
- The coordinator will lead this team through this change and support the positive integration of the team into the broader unit and department, whilst also championing the work of the team, achieving operational targets, and ensuring quality outcomes for clients.
- This position is responsible for building the capacity of internal services to reflect on, identify and improve processes and models to support access of all eligible seniors to receive services through the Support at Home program.
About you
You will bring:
- Extensive knowledge of CHSP and HCP programs, as well as the aged care sector in general.
- Proven experience in engaging with and managing critical internal and external business relationships / stakeholders.
- Proven experience in developing and driving operational strategies to achieve funding targets and manage budgets
- An ability to champion quality and positive outcomes for the client
- Experience in leading teams, getting the best out of people and leading change
- Ability to work autonomously & manage competing demands
