Where you'll be working
Northern Sydney Virtual Care Service (VCS) is located in Douglas Building, Level 5, Royal North Shore Hospital, with care delivered virtually from an onsite office environment.
Patients are also able to present in person to our clinic or have the potential for a home visit to receive treatment and care for urgent health needs.
What you'll be doing
The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.
Assess, plan, implement and evaluate nursing care in collaboration with individual/s and the greater health care team to achieve quality patient care outcome.
Using a virtual platform the VCS Registered Nurse will provide supportive, person-centred care and assessments of patients in the community.
Within a Virtual Care Service model of care the Registered Nurse will undertake nursing assessments and interventions to their level and scope of practice, clinical assessment, planning, implementation, and evaluation to support patients within their home environment whilst having a clear understanding of clinical deterioration and appropriate escalation pathways.
They will participate in a collaborative care model, communicating effectively with patients/clients, carers and other healthcare team members to support outcomes which safely meet patient/client needs, and support acute hospital avoidance.
VCS nurses are required to be involved in monitoring of the health status of patients, as well as responding to urgent and episodic health needs.

Candidates will need to meet the following criteria:
- Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) and have a valid Australian Drivers Licence.
- Demonstrated effective interpersonal, verbal and written communication skills.
- Current clinical knowledge and clinical problem solving ability.
- A commitment to excellence in nursing practice and continuing professional development.
- Ability to work effectively within a multi-disciplinary team.
