About the role
Catholic Homes has an opportunity for a Wellness and Reablement Officer to join our Home Care Team based at our Wellness Centre in Guildford on a full-time basis.
Our Home Care team builds lasting relationship with our clients so that they may stay at home or get out into the community to do the things they enjoy.
This position will be responsible for the facilitation of physiotherapy and occupational therapy group activities at the Wellness Centre and individual activities within clients’ residences.
The successful candidate will be provided with ongoing training and support from the Executive Manager Home and Community Care, CHSP Team Leader, and the Home Care Physiotherapy and Occupational Therapy teams.
To be considered for this position you will have:
- Certificate III or IV in Leisure and Lifestyle, Allied Health Assistance or similar
- Experience as a Lifestyle Facilitator, Physiotherapy and/or Occupational Therapy Assistant in a community or Home Care setting
- Experience creating and implementing person centred activities for aged care clients
- Current C class drivers’ licence
- Excellent communication and interpersonal skills
- Intermediate computer skills
- Be a team player with the ability to multitask and prioritise duties, and the ability to work independently
- Willingness to work across different sites as required.
