Role Details
The Port Lincoln office is seeking a Youth Worker, Transition from Care to join their team.
The Youth Worker is responsible for:
- Enhancing the social and living skills of young people to facilitate their successful transition to independent living and reconciliation with their families.
- Identifying and developing community support systems and networks for clients and connecting them to appropriate services.
- Assessing and responding to the individual needs of young people transitioning from care and increasing the chances of successful intervention.
- Liaising with the case manager and other key stakeholders in the planning, implementation and monitoring of programs for young people to strengthen the support in managing the key issues that face them.
We are looking for a confident individual who has the ability to undertake casework, incorporating suitable assessment, life skill development planning and implementation and transition from care processes for young people.
You need to have the ability to develop effective working relationships and provide advice to management, staff at all levels and with a diverse group of individuals.
