The Aboriginal Health Project Officer CMCC contributes to co-design of the implementation strategy for the Continuity of Care Protocols Program (CCP Program) that addresses and reinformed the priorities of both Closing the Gap and Child and Maternal Health initiatives by focusing on how health system performance improvement can ensure that health care provided to Aboriginal people is well organised, relationships are maintained, and information supports Continuity of Care.
The achievement of strategic outcomes/targets under the Closing the Gap Agreement 2020, and SA Health's Flinders and Upper North Local Health Network and DHW strategic priorities for improving the health and wellbeing of Aboriginal people within their region.
The Aboriginal Health Project Officer will achieve this by facilitating and documenting strategic planning processes, the analysis of data and provision of key insights to inform decision making and working collaboratively with a broad spectrum of stakeholders to co-design solutions to identified systems, service, program issues/barriers or gaps.
