St Patrick’s Community Support Centre (St Patrick’s) is committed to working in partnership with other agencies to support individuals and families who are homeless or at risk of homelessness in obtaining stable accommodation, independence, and improved quality of life. We provide a range of services to support them on this journey, including emergency relief and meals, health and mental health services, drug and alcohol services, life skills training and a range of accommodation options.
Program Overview
Youth Place is a partnership between St Pat’s Community Support Centre and Youth Futures, funded by the Department for Community Services. Based in Fremantle, Youth Place will deliver services for young persons aged 15 to 25 years of age, who are homeless or ‘at risk’ of becoming homeless.
Position Intention
This role will support St Pat’s to better engage in a trauma informed and culturally safe way with young Aboriginal clients experiencing homelessness and/or transitioning into long term stable accommodation. It will provide support and guidance to clients via cultural case management practices under the direction of the Youth Place Team Leader.
Key Responsibilities will include:
- Delivery of quality youth focused, trauma informed and culturally safe services to Aboriginal young people.
- Experience in the delivery of culturally safe services to Aboriginal young people with complex needs including homelessness, disability, mental health, trauma and dual diagnosis.
- Demonstrated understanding of best practice in the delivery of effective, culturally appropriate support services, particularly in an Aboriginal homelessness context.
- Undertake client intake and assessment for eligibility for St Pat’s Crisis Accommodation program.
- Provide case management to clients, including planning, support, informal counselling, referrals and linking to services that match unique needs.
- Facilitates group information session and workshops.
- Assist program participants to set and achieve short- and long-term goals using a person-centred approach.
- Organise and facilitate regular social and living activities for clients, encouraging the development of independent living skills.
Essential Criteria
- Aboriginality is a requirement of this position, as identified under Section 50D of the Equal Opportunity Act 1984 (WA)
- Experience in cultural case management/community work
- Tertiary level Qualification or relevant industry experience. (e.g., youth work, community services)
- Appropriate First Aid certificate or willingness to undertake first aid training.
- National Police Clearance.
- Working with Children Check if requested in line with agency policy and procedures.
- Possession of a current C class motor vehicle driver’s license.
