About the opportunity:
The role is highly administrative role, supporting the management in Support Coordination, provide assistance in coordinating the services and Supports for National Disability insurance Agency. The role of the administrative Support Coordinator is to support National Disability Insurance Scheme (NDIS) Participants to achieve their NDIS Plan goals, by coordinating. strengthen participant’s ability to navigate the NDIS and to facilitate implementation of all supports identified in a participant’s plan. This position works to a standard consistent with the guidelines set out by the National Disability Insurance Agency (NDIA). Connecting with family, friends and other community groups or networks.
Responsibilities include:
- Achieve targets as identified by the Manager of Support Coordination, in line with the strategic business plan and guidelines as set out by the NDIS
- Develop trusting, positive, and professional relationships with Participants and service providers.
- Provide information to Participants and advocate when necessary to facilitate access to community services and facilities.
- Undertake HR administrative duties as required,
- Maintain NDIS and high care documents on a regular basis and/or as needed.
- Completing and maintaining participant risk assessments, support plans and person-centred plans.

- Maintain relevant documentation and matching registers.
- Serve as the primary point of contact for participants
- Support participants in decision-making and coordinate a program both, in the community and in the home.
- Demonstrate excellent customer service skills when working with participants, support workers and other team members.
- Perform administrative tasks relevant to your works.
