About the role:
You will be responsible for coordinating the day to day operations of the service and providing professional culturally safe case management through referral, assessment and co-designed care planning for the frail aged and people living with disability in East Arnhem Land.
To be successful in this role, you will possess the following:
- A minimum Cert III in Individual Support, or the willingness to obtain within the first three (3) months of employment.
- A Diploma of Community Services (Case Management), or the willingness to obtain within the first eighteen (18) months of employment.
- Demonstrated knowledge and understanding of Aged Care and NDIS legislation, program guidelines and person centred care management principles.
- Proven ability in case management, assessment and care planning, meeting client/participant goals and preferences
- Proficient skills in Microsoft Office Suite and experience in electronic client management systems and databases.
- Experience in the use of Aged Care and Disability electronic platforms, client management systems and relative Government portals.
- Previous experience coordinating people / small teams
- Advanced oral, written and interpersonal communication skills, including effective stakeholder engagement, active listening and experience in navigating conflict and challenging situations.
- The ability to work with and foster teams that are inclusive and free to exchange ideas and actively contribute towards achieving service objectives.
- Proven ability to assess complex problems, develop a range of options and to make recommendations for solutions using creative thinking.
- Must hold a current manual C Class Driver’s License
It would be desirable if you also possessed:
- Previous experience living and/or working in a remote First Nations community within Australia.
- Demonstrated high level of cross-cultural awareness to ensure effective participation in a cross-cultural workplace.
- Awareness of issues affecting First Nations People in Australia,
The preferred applicant will be required to undergo a Criminal History Check and obtain a Working with Children Clearance. Criminal history will not exclude an applicant unless it is relevant to the position.

What you'll get in return:
Full Time Permanent employment.
Level 6 - Salary $87,190.50 - $92,500.36 (excluding super and locality allowance) with annual wage increase opportunities built into our EBA.
You will also benefit from annual leave with 17.5% leave loading.
