- Salary: $102,641 to $110,094 - AHP3 - Full Time / Term Contract (Temporary up to 30 June 2027)
- The Allied Health Clinical Project Manager - Health/NDIS Interface will provide expertise to support the coordination, development, management and evaluation of the strategies for transition of patients in state programs who have completed their post-acute phase of care however are delayed in discharge into ongoing service systems such as Aged Care or the National Disability Insurance Scheme.
- The Allied Health Clinical Project Manager - Health/NDIS Interface will also facilitate patient flow systems to avoid hospital admission and support early discharge by appropriate use of out of the hospital strategies/programs and associated demand management services to enable patients to receive care in the right place at the right time.
- The Allied Health Clinical Project Manager - Health/NDIS Interface is an expert within their area and stream of practice and has a significant degree of autonomy and decision making.

Criminal History Screening Requirement
- A National Police Certificate (NPC) for employment involving 'Unsupervised Contact with Vulnerable Groups' is required for this position (E.g. aged care employment or work involving vulnerable clients)
- A DHS 'Working With Children Check' (WWCC) is required for this position
SA Health Services are required to implement the in the workplace.
Special Conditions
- Appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers, or
- Appropriate degree or equivalent qualification and general registration with the Psychology Board of Australia, or
- Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
