This position is a permanent part time position. The role provides case management / care coordination to clients in a community setting on Transition Care, Restorative Care and GEM in the Home programs. These 3 programs form the Ambulatory Aged Care Programs.(AACP)The Clinical Care Coordinator is an integral part of the multidisciplinary team. They will work in consultation with Physiotherapists, OT's, Team Assistants, Speech pathologists, Dieticians, Geriatricians and Team leaders to implement best practice for AACP clients.This position will provide the successful applicant with valuable Case Management/Care Coordination skills which is a sought after quality in community health programs.The successful applicant will have:
- Current Registration with the Australian Health Practitioner Regulation Agency in the relevant discipline (if applicable).
- Relevant Tertiary Qualifications in Nursing or one of the Allied Health disciplines and eligibility for professional body membership.
- Minimum of three years clinical experience and the ability to practice flexibility across acute, sub-acute Aged Care and community sectors.
- Demonstrated ability to case manage clients and coordinate services within a care coordination model.
- The ability to work collaboratively as an individual practitioner within an interdisciplinary team environment including the ability to initiate and maintain effective relationships.
- Well-developed understanding and experience of allied health and nursing practice delivered in the rural setting, particularly within a regionally integrated service delivery model.
