AddressPurpose of position
This role works closely with the AN-ACC Manager and on site clinical, allied health staff and management, to maintain assessments, maximise AN-ACC funding and review processes to achieve best possible care outcomes.
Key Accountabilities
- Work with staff to improve (AN-ACC) processes and provide staff training and support in respect to the (AN-ACC) funding instrument.
- Identify opportunities and provide education / training for staff in relation to (AN-ACC) documentation.
- Liaise with staff to ensure the information contained within the database for the resident is accurate and up to date.
- Ensure resident assessments, initial care plans / reviews and other relevant clinical documentation is completed accurately and reflects the individual resident care needs and legislative requirements.
- Identify opportunities for maximising (AN-ACC) reclassification funding for individual residents when care needs change in discussion with the relevant Site Manager, Clinical Staff and Allied Health Professional.
- Ensure assessments accurately support funding claims and reclassifications.

Essential Requirements
- Police Check
- Pre-employment medical assessment
- Current AHPRA Registration
- Current QLD Drivers Licence - Travel between Sundale sites is a requirement for this position
Aged care or health industry experience with AN-ACC knowledge would be highly desirable, training will be provided to the right applicant.
