About the role

Eastern Health is currently looking for a passionate and experienced Pharmacist to join our Pharmacy Department to establish and lead the Anticoagulation Stewardship Program. Eastern Health operates five pharmacy departments, located across the eastern region of Melbourne. Our pharmacy department strongly values professional development and clinical education. The position is responsible for establishing an Anticoagulation Stewardship Service at Eastern Health to monitor anticoagulant use, develop policies and procedures, in addition to education and training packages. The role will work collaboratively with the Anticoagulation Stewardship Medical Lead, Haematology Unit and the Pharmacy Program to provide governance to ensure anticoagulant prescribing practices are consistent with current evidence for safe, quality medication use. The position will be responsible for driving improvements in quality and safety of anticoagulant use. This member of the EH Pharmacy management team and will manage the Anticoagulation Stewardship Pharmacy Technician.

About you

To be successful in the role you will possess:

• Registered or eligible for registration as a Pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA)
• Current and advanced hospital pharmacy experience, including effective clinical pharmacy skills
• Demonstrated ability to perform under pressure, to meet time constraints and determine work priorities and a high level of organisational skills
• Demonstrate ability to work as part of a diverse multidisciplinary team
• Proficient computer and data management skills (including intermediate knowledge of Microsoft Office programs)
• Experience in Stewardship Service development or delivery, particularly experience in Anticoagulant Stewardship is highly desirable
• Active participation in the affairs of relevant professional associations which may include membership of Special Interest Group, Specialty Practice Leadership Committee or similar relevant to portfolio is highly desirable
• Demonstrated ability to manage projects and to meet deadlines
• Highly developed interpersonal experience
• A strong commitment to high quality care
• Excellent communication skills
