Sovereign Lives Victoria assists with the delivery of care and support services to people with disabilities to continue to live independently at home and community.
We are passionate about supporting people with disabilities, and that means providing advice, empathy, and understanding to ensure that our amazing participants have the support they need to live happy and healthy lives.
About the role:
Care Coordinators plan and coordinate the delivery of an organisation’s services. They build and maintain relationships with Disability Support workers, NDIS participants, families, allied health, and other stakeholders to continuously improve the level of service by ensuring that the participants' requirements and organisation's goals are met. Care Coordinators are highly organised and have strong administrative and communication skills. They interact with participants daily and use a range of computer software. They act as an advocate for the NDIS participants and provide information on how they can access the community services. In additional, they work as an intermediary between participants, often serving in a support role for them during difficult times.
About the Role:
Here’s what you can look forward to as a Care Coordinator with us:
- Working directly with your clients, family members, and their supports to identify their needs and ensure that the support provided meets them.
- Updating Care Plans and Budgets for our clients to ensure that their care needs and deterioration of health are well documented.
- Support from your Manager as well as your team members to achieve goals.
- To contribute in building a warm and friendly work environment.
- To have existing knowledge about public health and well-being
- To conduct and attend meetings and training sessions
- To educate people with disability and family members/health care workers about health, safety and quality living standard
- To prepare care plans/reports of NDIS participants and brief health care workers
- To conduct training and briefing sessions
- To work under the management of the Senior Management members
- To work in a team environment by diverting or receiving diverted phone calls during colleagues' absence
- To promote the services of the organisation based on your network building and excellent care coordinating services
- To prepare and write weekly/fortnightly or monthly reports of participants' services
- To write progress notes in our established CRM and system
- To provide accountability of accomplished/unaccomplished tasks, exchanged phone calls, messages and progress with all participants.
- To work in the best interest of our NDIS participant
- To maintain privacy and confidentiality of the organizations
- To work according to the NDIS Code of Conduct
- To take immediate actions and pay attention to all matters concerning the supports of the NDIS participant
- To coordinate with the rostering team to ensure that the participants are receiving supports as and when required
- To maintain excel of all compliant documents (incident forms, PCP, ERM, Home Risk assessment and more)
About you:
This position would be suitable for someone with medical background/public health, or someone with experience in residential and/or community care.
However, we value a positive and enthusiastic attitude and will train the right person.
Here’s the type of person we are looking for:
- An enthusiastic individual who understands the importance of helping people and is passionate about providing excellent service.
- Relevant aged care/health qualifications (For example Aged Care, Disability, Social Work, and/ or Community Services/Public Health qualifications)
- Diploma in Community Services or other healthcare courses
- Excellent written and verbal communication
- Relevant work experience in the Disability services sector
- A caring and professional nature.
- To work on a probationary period until the organization is satisfied with the performance and level of interest in the job
Desirable:
- Care Management experience.
- An understanding of NDIS
- Qualification in Community Services
