AddressOn offer is an exciting opportunity for an experienced Care Manager to work for a successful Aged Care provider. This is a large home that requires a proactive and energetic clinical manager who takes pride in delivering excellent resident care.
This is a highly collaborative role requiring you to confidently showcase your strong communication and interpersonal skills to clinical team, residents, and families.
You bring with you at least 2 years experience in a similar role; working with an experienced team, now is your chance to strengthen your leadership skills and clinical knowledge.
Key responsibilities include:
- Ensuring the groups quality standards are being upheld within the facility by consistently being aware of gaps and working on solutions.
- Residents care plans and needs are upheld at all times by overseeing RNs daily operations and time management.
- A passion for developing teams and maintaining a great team culture.
- Working collaboratively to provide an exceptional customer experience to internal & external stakeholders.
- Registered Nurse - AHPRA registration.
- Previous Care Management experience
- Solid understanding of all Aged Care accreditation standards
- Demonstrated knowledge of all relevant legislation, ACFI, regulatory requirements and professional standards
- Excellent relationship-building skills with the ability to lead & mentor teams
- Solid communication and presentation skills.
