As Care Manager you would have the opportunity to provide high-quality, client-centred care for older adults in their homes across South West Victoria. You'll work closely with clients, families and service providers to develop personalised plans that promote independence and well-being.
In the role you'll:
- Create care plans and coordinate services aligned with client needs and budgets.
- Advocate for clients from diverse backgrounds, ensuring a culturally sensitive approach.
- Develop creative solutions to address complex clinical and social challenges.
- Collaborate with healthcare professionals and service providers to ensure seamless care coordination.
- Promote La Bella Life services within the local community.
- Maintain accurate client records and budgets.
- Contribute to continuous quality improvement initiatives.
- Uphold ethical and professional standards.
What you bring:
- Experience in health care and case management.
- Strong knowledge of the healthcare system, community resources and aged care regulations.
- Excellent communication, interpersonal and problem-solving skills.
- Compassionate and client-centred approach.
- Ability to work independently and collaboratively.
- Commitment to continuous professional development.
- Proficiency in ICT applications and basic budget management.
- Current driver's license and satisfactory police check.
- Reliable vehicle with full comprehensive insurance.
- Must be located in South West Victoria.
