What is the opportunity?
We have two exciting opportunities for motivated, self-directed and experienced clinicians to join our team as Care Managers!
Position 1:Backfill the role of one of our Care Managers while they are on a period of parental leave for 12 months. Role commencing in March 2024 (potential for this to be an ongoing at the conclusion of the 12 month period).
Position 2: Permanent, full-time Care Manager position needing to be filled on an ongoing basis.

About the Care Manager role
- A Registered Nurse or Endorsed Enrolled Nurse by profession, the Care Manager is a key role responsible for the provision of care and services to a select group of valued clients in and around Melbourne.
- This rewarding and hands on role requires a proactive approach to the planning, coordination, delivery, and case management of our clients’ care and services.
- Working closely with the Rostering Team, the role includes engaging with and leading our care team, consisting of Nurses and Carers to provide the best care possible to private, HCP, NDIS and DVA clients.
- Working with Care RN’s and an After Hours team that covers weekend and weeknights on-call.
- Full-time, maximum term position, based at our office in Hawthorn, working with clients in the eastern and northern suburbs of Melbourne.
Role requirements
- Unrestricted registration with AHPRA as a Registered Nurse or Endorsed Enrolled Nurse.
- A minimum of 3 years recent clinical, care or case management experience in an aged care or community setting.
- A current full drivers license and a well maintained, registered vehicle.
- The right to work in Australia & ability to obtain a satisfactory Police Check.
