About the roleThe
Transition Care Program provides older people with time and support in a non-hospital environment, to enable them to complete their restorative process, optimize their functional capacity and finalize and access their long term care arrangements. The
Case Manager assists clients and their family/carers in adjusting to changes in health status and facilitates the establishment of community supports and services and, where required, identification of residential care options. The Program’s clinical staff provide low-intensity therapy and other services to maintain/improve clients physical and cognitive functioning and to facilitate improved capacity in activities of daily living.
About youTo be successful in this role you should possess:
- A recognised tertiary qualification in a health-related discipline.
- Registration and ongoing registration requirements
- Nursing/OT/PT - Current registration with AHPRA and ongoing Mandatory registration annually
- Other Allied Health – Eligibility for practising membership of relevant professional body – either evidence of current membership or completion of a statutory declaration which specifically affirms they meet each of the current criteria set for membership. Those who obtain their qualification overseas must have the professional body formally confirm their eligibility for practising membership.
The skills and attributes you will need:
- Knowledge of relevant aged care community and residential care resources.
- Extensive experience in service provision to elderly people with complex needs to ensure effective treatment and discharge planning.
- Effective interpersonal, organisational, problem solving and decision-making skills.
- A commitment to working within a multidisciplinary team
