About the role
As a Care Manager, you'll play a pivotal role in enhancing the quality of life for our consumers. Your responsibilities will include:
- Conducting thorough assessments and developing comprehensive care plans.
- Preparing budgets and identifying tailored care solutions.
- Managing significant incidents and handling consumer feedback.
- Overseeing continuous quality improvement initiatives.
- Ensuring compliance with internal policies, governance, and statutory requirements.
- Collaborating with the leadership team to achieve strategic business goals.
About you
We're looking for someone with a strong background in the Aged Care Industry and demonstrable experience with Home Care Packages (HCP). You should have:
- Expertise in developing detailed consumer assessments and care plans.
- Frontline management skills and the ability to oversee significant incidents.
- A commitment to continuous quality improvement and compliance.
- Excellent collaboration skills to work with our leadership team.
Qualifications & Experience
- Qualifications in Nursing, Social Work, Community Services, or a related field.
- A valid driver’s license, vehicle registration, and comprehensive car insurance.
- National Police Clearance.
