Role Overview:
As a Care Manager, you'll be at the forefront of our mission, leading the coordination of care for our clients. Your responsibilities include building strong relationships, developing personalised care plans, ensuring compliance with standards, and providing leadership in care management.
Key Responsibilities:
Client Relationship Management: Establish and maintain strong relationships with clients and families, ensuring the highest quality of care through regular interactions.
Care Planning and Management: Lead service inquiries, consultations, onboarding, and reviews. Collaborate with clients and families to develop tailored care plans.
Leadership and Coordination: Provide leadership in managing and reviewing client care goals. Ensure effective communication within the team and with clients to meet expectations.
Documentation and Compliance: Maintain accurate records and ensure compliance with relevant standards and guidelines.
Requirements:
Qualifications and Experience: Relevant qualifications in care planning or management. Experience in aged care, community health, or social services is essential.
Skills and Attributes: Strong communication, problem-solving, and time management skills. Highly organised and capable of working under pressure.
Licensing and Technical Proficiency: Valid Australian Driver’s License, IT proficiency, and ability to obtain a National Police check.
