The Complex and Restorative (CARE) Service: The Southern Adelaide Local Health Network (SALHN) and SA Ambulance Service have partnered together to develop an integrated, multidisciplinary, geriatrician led service which has reshaped how non-emergency care is delivered to the older person in the Southern Adelaide catchment.
The CARE Service will provide care to older people with complex medical, functional and/or psychosocial needs with the goal of 'home first', via assessment, triage, diagnostic or direct care for streams within the CARE service. Delivery of the service may be provided in the CARE centre, located at Repat Health Precinct, or via an 'Eyes on Scene' outreach team in the consumer's home.
Key Responsibilities:
- Provide direct expert nursing care including assessment, triage, diagnostic procedures, and direct care within the CARE service streams.
- Collaborate with SAAS staff and leadership within the Command Centre for real-time assessment and referral of alternative pathways.
- Deliver multidisciplinary acute and restorative treatment in consumers' homes as a member of the Eyes on Scene Outreach Team to prevent unnecessary hospital transfers.
- Lead and deliver comprehensive geriatric assessment as part of a multidisciplinary team focusing on complex and restorative care within the Care Centre.
To be successful in this role, you will have:
- Demonstrated expertise in multidisciplinary assessment and triage decision-making skills.
- Experience in providing direct expert nursing care and leadership within a healthcare setting.
- Strong understanding of geriatric care principles and complex discharge planning.
- Ability to contribute to clinical practice research and service improvement initiatives.
- Excellent communication, collaboration, and conflict resolution skills.
