We are looking for suitably qualified and enthusiastic Case Coordinators to join our Complex Needs Coordination Team - Mental Health Expansion (CoNeCT MHE).
This is a Recruitment Pool to fill Fixed Term Full Time and Fixed Term Part Time appointments, with the possibility of extension(s) or permanency.
Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 30/04/2025.
Case Coordinators provide an advanced level of hospital and community focused care coordination to an identified “at risk” client target group of people and their carers’, whose care and support needs are assessed as complex.
This role is responsible for being a metro-wide resource on care coordination especially in the areas of mental health and disability. You will network and link with key stakeholders across the care continuum of acute care, sub-acute care and community settings. This will include education and liaison to actively build relationships across services working with the client group.
The Case Coordinator manages a client caseload and apply specialist knowledge in coordinating the provision of care to the client and the client's carer(s). You will also be responsible for the following:
- Clinical: Work within a multidisciplinary team to provide hospital and community focused care coordination to an identified “at risk” client target group of people and their carers’, whose care and support needs are assessed as complex. Responsible for being a metro-wide resource on care coordination especially in the areas of mental health and disability.
- Education, Liaison and Networking: To network and link with key stakeholders across the care continuum of acute care, sub-acute care and community settings. This will include education and liaison to actively build relationships across services working with the client group.
- Evaluation and Reporting: To actively engage in development and implementing of an evaluation process, including recording, collecting and reporting key performance information. Conducts research activities in line with the CCC program development and future planning to ensure evidence-based practice healthcare.
