Case Manager - Child and Parenting Support

About the role:
We are currently seeking a Case Manager - Child and Parenting Services to join our Family and Relationship Services team. In this role you will be providing support to children and parents through case management, home visits, group work and supported playgroups using an integrated service delivery model.
This position is a permanent part time 3 days per week, Tuesday – Thursday based in Liverpool, NSW.
Key Responsibilities (not limited to):
- Ensures effective service delivery to clients to support best outcomes by advocating, establishing strong network links with local services to promote referrals pathways and developing strong partnerships with the church.
- Ensures that Integrated Service Delivery provides a quality client service by assisting in the development of innovative processes and systems that are regularly reviewed and evaluated.
- Ensure the service is well known in the community by developing resources that promote and advertise the service to clients, local forums and other community organisations.
- Ensures client eligibility for the service by conducting a thorough assessment process which includes home visiting and telephone assessment.
- Ensure individual needs of client are met by providing a case plan that is developed with the client, is clearly defined and with achievable outcomes reviewed every three months. The case plan must cover a range of case management, home visit, group work and supported playgroup options as relevant to client needs.
About you:
- Relevant tertiary qualifications e.g. Social Work degree, Social Sciences, counselling, Early Childhood, social welfare or equivalent
- Case management and education experience with a strong client focus on the issue of parenting and child development.
- Ability to lead and facilitate groups to educate and support clients with evidence based programs.
- Ability to communicate effectively with clients from different and varied backgrounds, including CALD and Aboriginal communities
- Excellent written and verbal communication skills
- Current NSW Drivers licence
- Current paid Working with Children's Check (WWC)
- Covid – 19 vaccination is strongly recommended
