We are seeking a passionate Case Manager to work together within the team as key members in growing the business.
Our Case Manager ideally has a healthcare/marketing/business background . They are passionate about ensuring clients have access to safe and appropriate care and environments.
Main responsibilities include:
- Drive business development and account-related activity that supports the business’ growth aspirations
- Representing Assure Ability in a positive and responsive manner
- Conduct comprehensive assessments of clients' needs, strengths, and resources to develop individualized care plans.
- Coordinate and facilitate access to community resources, including healthcare, housing, employment, education, and social services, to meet clients' needs.
- Advocate for clients' rights and needs within the community and with other service providers, ensuring they receive appropriate and timely support.
- Regularly monitor clients' progress towards goals outlined in their care plans, evaluate effectiveness of interventions, and make adjustments as necessary.
- Provide crisis intervention and support to clients facing emergencies or difficult situations, connecting them with appropriate services and resources.
- Maintain accurate and up-to-date client records, including assessments, progress notes, and service plans, ensuring compliance with organizational and regulatory requirements.
- Work collaboratively with interdisciplinary teams, including social workers, healthcare professionals, and community organizations, to ensure holistic care for clients.
- Provide education and support to clients and their families on relevant topics such as health management, financial literacy, and life skills development.
- Engage in community outreach activities to raise awareness about available services and advocate for systemic changes to improve the quality of life for individuals and families.
- Stay informed about current trends, best practices, and resources in the field of community services through ongoing training, workshops, and professional development opportunities.
- Work within relevant funding guidelines and agreements, legislative requirements and organisational policies and procedures to produce best practice results for the clients we assist.
Requirements:
- Previous experience in a role of similar nature, including responsibility for the management and operational oversight of a Disability Service supporting clients with complex and differing needs
- Qualifications in Disability or Community Services will be highly regarded
- Commitment to the provision of exceptional customer service
- Passionate about the work and have patience and mentality to deal with Disable People specially who are mentally challenged
- Extensive experience in the human services sector with an in-depth knowledge of NDIS, industry standards, quality frameworks and legislative requirements
- Excellent organisational and time management skills with the ability to prioritize and manage multiple and competing work tasks and deliver to agreed deadlines
- Advanced computer skills, including the use of Microsoft Office and a range of online systems.
- Unrestricted NSW Driving License
- NDIS Worker Screening Check and Working With Children Check
The ideal candidate will also have either of the following:
- Minimum 6-12 months experience working in the Disability industry
