We are a growing Home Care Package provider that is looking for an experienced Case Manager to join our team, focussing on empathy, kindness and providing excellence in care to our beautiful community of elderly recipients living in their own homes.

Tasks and Responsibilities:
- You will be an experienced person that can provide comprehensive client assessments and develop care plans to support consumers in their own homes;
- Provide holistic care skills within a case management framework;
- Consult, network and liaise with the rostering staff and relevant agencies, contractors, service providers and key stakeholders;
- Contribute to the ongoing development of the Home Care Program, including participating in development of procedures, planning and reviewing of care recipient services;
- Meet weekly with the Care Manager and Team Leader to provide reports on achievement of KPI's and budgets;
- Liaise with multiple service providers to ensure care recipient needs are met in line with the client care plan;
- Monitor and review ongoing consumer needs, physical and emotional wellbeing and make appropriate recommendation of supports and services;
- Actively respond to new customers, manage referrals, client details and upgrades to consumers on the My Aged Care Services Portal;
- Analyse client issues, problems solve to achieve realistic and practical client centred solutions;
- Keep up to date with reforms, the revised aged care act, legislation, the Home Care package manual - Inclusions and exclusions and the new and improved quality standards and be able to apply them in your day to day work with consumers;
- Understand and prepare consumer budgets;
- above all, be proactive and client focused.
About You:
- You will be passionate about making a difference to the lives of our consumers living in their own homes, and provide outstanding customer service, and:
- You will possess excellent communication skills (both written and verbal communication);
- Tertiary qualification in a health related field e.g. nursing in aged care (Home Care) case management aged care, social work, allied health.
- Possess a broad knowledge and experience of the Aged Care Quality and Safety Standards;
- Customer focused with a high focus on communication and problem solving;
- Ability to take the initiative and pursue opportunities and optimise resources;
- Flexibility, adaptability and a good team player;
- Good interpersonal, analytical and client assessment skills;
- Ability to manage and monitor consumer expenditure and budgets;
- Good stakeholder engagement and relationship building skills;
- Current Victorian drivers license, Current national police check, current vaccination status in line with government policies for health care workers.
- Have a sense of humour.
