Case Manager - Ndis (Maternity Cover With Opportunity To Extend)

About us

Careseekers is a NDIS provider and digital platform connecting individuals, families and organisations with aged care and disability support workers across Australia. Our goal is to support older Australians and people with disabilities to live life on their own terms and find meaningful job opportunities for aged care and disability support workers.

In this Case manager role you will:

• Support NDIS participants to connect with independent support workers through our platform. This involves participant onboarding, management and reviews of participant goals and outcomes.
• Support NDIS participants who have restrictive practices in their behaviour support plans.
• Support NDIS and Home Care Package recipients as they manage their in-home supports and services.

Please apply if you have:

• Demonstrated experience in a similar role e.g. case coordinator/manager, support coordinator in the NDIS sector. Minimum 2 - 3 years experience.
• Experience supporting NDIS participants with complex needs.
• Comprehensive knowledge of, and experience working in the NDIS system and adhering to NDIS policies, protocols and procedures.
• Demonstrated participant centred focus.
• High attention to detail, with the ability to multi-task and complete documentation and administrative work in a timely manner.
• Excellent written and verbal communication skills.
• Demonstrated knowledge and experience in successfully working as a team
• Commitment to the provision of exceptional customer service

Role is nine months initially (maternity leave cover) with opportunity to extend.
