Our team provides a range youth services, including Housing and Homelessness, youth justice responses, training, and general supports to assist young people to address challenges in their lives and make positive change through a mix of practical and therapeutic interventions and the creation of opportunities for positive social participation.
About You
You are a dynamic, passionate, experienced youth Housing and HomelessnessCase Manager who provides a high standard of client led case management and group activities with vulnerable, at-risk young people aged 16 – 25.
As a Case Manager you will work with young people in our Housing and Homelessness program to identify their needs and develop a plan to deliver targeted and goal-driven supports that focus on the individual young person’s needs and aspirations. You will facilitate groups and life skills workshops for clients.
You will undertake a range of record and information management tasks including completion of case notes, preparation of reports, performance reporting and data entry to ensure casework and accountability.
You will have extensive knowledge of social Housing and are a wizard at sourcing alternative accommodation. You have great communication and are enthusiastic about liaising with internal and external stakeholders for the purposes of enhancing and discussing case planning and building community relationships.
The successful applicant will support the organisation’s strategic objectives to increase the range of programs and services offered to young people, continue to evolve practice, and increase young people’s participation in the development of such initiatives.
Group work, Youth Housing and homelessness experience is desirable.
