Role Overview: As a Client Liaison Coordinator, you will join our Client Engagement team, an essential complementary service for our clients. Based at our Wilson services centre, your role will involve managing a case load of NDIS Clients.
Your responsibilities will include:
- Building Rapport: Establishing strong relationships with clients.
- NDIS Plan Oversight: Ensuring clients’ NDIS plans and budgets are effectively managed.
- Assessments: Conducting assessments to identify clients’ needs and goals.
- Comprehensive Services: Providing advocacy, planning, referrals, and yearly review reports.
- Individualised Support Plans: Collaborating with clients and their support networks to develop personalised support plans.
- Service Coordination: Identifying and coordinating appropriate service providers and supports.
- Ongoing Support: Providing continuous guidance to clients and their support networks to achieve their goals.
- Progress Monitoring: Regularly assessing client progress and adjusting support plans as needed.
- Accurate Records: Maintaining up-to-date records of client progress and service delivery.
- Effective Communication: Engaging with participants, their families, service providers, and other stakeholders.
What You’ll Bring:
- Disability Services Experience or Qualifications: Ideally, you should have relevant experience in disability services.
- NDIS Funding Knowledge: Familiarity with the NDIS is crucial.
- Strong Interpersonal and Communication Skills: Effective communication is essential.
- Passion for Making a Difference: A genuine commitment to positive outcomes.
- NDIS Worker Screening Check: Or willingness to obtain.
- Current Australian “C” Class Driver’s Licence with no restrictions.
- Excellent Time Management and Organizational Skills: Vital for a busy environment.
