Alzheimer's Queensland Multi-Service Centres provide a restorative and wellness approach to support older people stay engaged within their community. Our Multi-Service Centres are based in home-like cottages in order to evoke feelings of comfort, relaxation and personability, familiarity and independence.
An opportunity has become available for a highly motivated Full Time Client Services Coordinator to play a crucial role in ensuring the seamless delivery of care services to our clients at the Toowoomba Respite Centre.
Your key focus is the building of partnerships with both clients and their families as well as the provision of staff coordination and support to ensure the best possible care is delivered to our clients. We believe in creating a warm and supportive environment where seniors can thrive whilst receiving the care they deserve. Other key responsibilities:
- Client communication
- Client care planning and assessment
- Effective client induction
- Problem resolution
Requirements for Success:
- Minimum qualification of Cert IV in Aged Care or Case Management or community services
- Previous experience in Client Services coordination, preferably in the aged care sector.
- Strong interpersonal and communication skills.
- Excellent organizational and multitasking abilities
- Compassionate and empathetic approach towards seniors and their families.
- Proficient in relevant software and office applications
- Excellent planning, time management & problem-solving skills
- Driver's License, National Police check and covid vaccination
- Budget management & exposure to rostering
