An exciting opportunity currently exists for a client-focused individual who will be responsible for leading a team of care coordinators and registered clinical providers. You will be responsible for overseeing the delivery of Clinical Care and coordination of services that results in achieving client goals and desired outcomes. Although we are located in Port Macquarie applicants from other areas are encouraged to apply. Salary is negotiable for the right candidate.
Responsibilities include
- Provide Clinical Care to our clients as well as providing clinical expertise to Care Coordinators and employees.
- Use a systematic multiple approach to clinical decision-making
- Implement and monitor the care of clients and their carers
- Collaborate with and lead the health care team to achieve client outcomes
- Act to enhance the professional development of self to align with professional standards
- Function in accordance with legislation and regulatory requirements
- Ensure the Aged Care Standards and all aspects compliance are met and adhered to
About you:
- Sound understanding of the Aged Care and NDIS Standards
- Understanding of, and a commitment to, person centered and strength-based practices
- Registered nurse with current APHRA registration
- Sound clinical knowledge and experience
- An innovative and dynamic approach to the provision of care
- Excellent organisational and time management skills, with a high attention to detail and ability to establish priorities
- Excellent interpersonal and communication skills – written and verbal
- Ability to follow policies and procedures and implement quality processes
- Demonstrated understanding of, and commitment to, work health and safety requirements
- Previous clinical experience in aged care or community setting and/or Catheter management and wound management experience would be highly regarded.
The successful applicant will need (or be willing to obtain):
- Current First Aid and CPR Certificate
- Current APHRA Registration
- Current Driver's Licence
- Working with Children Check
- NDIS Worker Screening Check
