This is a fantastic opportunity to join our dynamic team as a Clinical Coordinator at Bene Home Care. You will play a vital role to co-design, refer and coordinate the delivery of services to clients with complex care needs. This is a key leadership position within our team as we continue to grow our Home Care services.

What will you be doing?

• Conduct comprehensive assessments and reviews of client goals, needs, and preferences.
• Develop complex care and behavioral support plans in collaboration with Case Managers.
• Partner with Case Managers, clients and families to support identified care needs.
• Identify and refer clients to internal or external providers, as necessary.
• Provide education programs related to clinical assessment and practice, including medication credentialing.
• Stay up to date with best practice guidelines and provide clinical supervision to Registered Nurses as required.
• Lead clinical quality improvement initiatives across Home Care, including participating in clinical audits.
• Provide guidance and support to clients and families navigating the aged care journey.
• Contribute to the oversight and investigation of high-risk client incident reports.

What are we looking for?

• Full, unrestricted AHPRA registration and unlimited work entitlements
• Working knowledge of the principles of wellbeing, reablement, and restorative care.
• Innovative and creative approach to service delivery.
• High level of written and oral communication skills.
• Demonstrated knowledge of Work Health and Safety requirements.
• Experience and knowledge of the Aged Care or Disability Industry.
• Current National Police Certificate and Driver’s License.
• Evidence of right to work in Australia.
