Fantastic Opportunity for an experienced Care Manager to join this well-established aged care organisation minutes from Perth’s CBD.
We have an individualised approach to resident care so the ideal candidate will be responsible for ensuring our residents are provided with high quality care and that their needs are being met both efficiently and effectively.
This role will also see you directing and mentoring our wonderful team of nurses and support staff to maintain this high level of care, so we are ideally looking for someone with a strong clinical and leadership background who has the ability to drive continuous improvement within the home.
Key Responsibilities:
- Support residents and their families by efficiently addressing queries, comments, suggestions, or concerns.
- Deliver on all clinical governance reporting and requirements.
- Maintain and adhere to quality systems and processes to ensure our home is compliant with all standards, as mandated by ACSQC.
- Ensure care provided to every resident is evidence-based and best practice and reflected in accurate documentation with appropriate strategies implemented.
- Nurture and support our nursing and support staff along with our beautiful resents and their families
Qualifications:
- 2-3yrs previous experience in a Care manager role within an aged care setting
- A strong focus on continuous improvement
- Solid understanding of the aged care quality standards and ANACC funding
- Current NDIS clearance, valid working rights in Australia and Covid and Flu vaccinations up to date
