About the Role:
Exciting opportunities exists for experienced Clinical Nurse Project Officer to join our Logan Hospital team!
As the Clinical Nurse (CN) Project Officer, you will be responsible forproviding advanced clinical and problem-solving skills, planning, and coordination, in the management and facilitation of respiratory patients to be safely discharge to their General Practitioner (GP). The CN project role is aimed at facilitating the safe discharge of COPD patients, to ensure consumers are connected with their GP for ongoing care in the community.
