Position Profile: We are seeking a skilled and experienced nurse to provide clinical support to the Rheumatology Service based at PCH. The CN is a key part of the multidisciplinary team and will contribute to delivering outcomes consistent with the service and organisations values.

The successful applicant will deliver care and support to patients and their families in outpatient clinics, via telephone and virtual consultations and in inpatient settings. They will have highly developed paediatric nursing skills and expertise to ensure appropriate evidence based holistic nursing care is delivered to all patients.

Other responsibilities and duties include the following:

  • Act as an advocate for patients and families and collaborates with them and other members of the health care team to achieve health goals.
  • Demonstrate competence in the planning and delivery of complex care to patients and their families.
  • Co-ordinates the daily activities of the unit/ward.
  • Completes nursing documentation as per the health service/hospital standards and policies.
  • Participates effectively as a member of the multi-disciplinary team and fosters continual development of the team to provide expert services.
  • Supervise and acts as a resource for staff in the assessment of patients, development of care plans, implementation and evaluation of nursing care.
  • Facilitate the development of staff in clinical competencies, organisational skills and participating in their performance review.

CAHS is made up four service areas: Neonatology, Community Health, Child and Adolescent Mental Health Services (CAMHS), and Perth Children’s Hospital (PCH).
